To register, please either send us a WhatsApp on +447893 950646 asking for an online registration form,

or email us at hello@manchestercatclinic.co.uk with the following information...

1. Your name

2. Your telephone number

3. Your full address

4. Details of anyone else who co-owns the cat with you and you'd like to share your account with, such as your partner, family member or flatmate (include their name, mobile number and/or email address).

5. The details of your cat(s), including:

   1. Name

   2. Sex (including if they are neutered if not)

   3. Date of birth (or approximate age if not known)

   4. Breed (or just if long or short haired if they are a moggy)

6. The name of your previous vet practice

7. Any other important information you'd like us to know about you or your cat.

Please note: Cats must be registered in the name of the person who actually owns them (ie. who they live with and who is responsible for their vet bills). If you are submitting details on behalf of someone else, please inform us at the time of registration.
